Directions To Our Location
We are located in Proctor Hospital Complex at 5401 N. Knoxville in Peoria. Our building is attached to the hospital in the Professional Wing. Entrance #2 or #4 will bring you into our hallway. We are on the main floor in suite #115. Please see our map and directions from main arteries into Peoria.
From I-74 heading west, take exit 92A (Knoxville Ave.).
From I-74 heading east, take exit 89 (War Memorial Dr./U.S. 150). After you exit, take the next exit right onto War Memorial Dr./150 east. Continue to Knoxville Ave. and turn north (left).
